Terms of Device Rental Agreement
This agreement governs the rental and use of the Luna device ("Device") provided to you by Cairns Health ("Company").
1. Device Ownership
The Device remains the property of the Company at all times. You are granted a non-transferable right to use the Device during your active subscription period.
2. Subscription and Usage
- You may use the Device for the duration of your active Luna subscription
- The Device is for personal use only and may not be sold, transferred, or lent to others
- You are responsible for the care and proper use of the Device
3. Return Requirements
Upon cancellation of your subscription, please:
- Return the Device within 90 days of your subscription cancellation date
- Ship the Device in its original or protective packaging to 530 Lakeside Drive, Suite 260 Sunnyvale, CA 94085
- Obtain tracking confirmation for the return shipment
We will provide a prepaid return shipping label and protective packaging upon request. Simply reach out to us via email help@cairns.ai or phone 650-206-9499.
4. Failure to Return Device
If the Device is not returned within 90 days of subscription cancellation, you authorize the Company to charge your credit card on file $750 as a non-return fee for the unreturned Device.
5. Damaged or Lost Devices
You are responsible for the Device while in your possession. Damaged or lost Devices may incur repair or replacement charges at Company's discretion. By accepting the Device, you acknowledge that you have read, understood, and agree to these rental terms.
Cancellation Policy
To cancel your subscription, please contact us at 650-206-9499 or email us at help@cairns.ai. We will provide a prepaid return shipping label and protective packaging upon request. Upon cancellation, please:
- Return the Device within 90 days of your subscription cancellation date (or you will be charged $750.00)
- Ship the Device in its original or protective packaging to 530 Lakeside Drive, Suite 260 Sunnyvale, CA 94085
- Obtain tracking confirmation for the return shipment
